Medical Certificates For Travel
If you are preparing to study, work, and or stay abroad for a long period of time you may have been asked to provide a medical certificate or a letter that is signed by a physician. If you are also a new resident to Canada and may need a medical letter attesting to your vaccination record Summit Health has now made this service available to our patients
Patients have the option to use Summit’s pre-formatted letter that will be signed by an MD, or we can create a letter specific to your requirements depending on what is required by your country or organization.
Please note a tuberculosis test is required, in order to complete the medical certificate. If you have completed one elsewhere, bring a copy of your results.
Step 1: Initial Consultation
An initial consultation will take place to review medical history, vaccination history as well as review the requirements for your certification

Step 2: Certificate Issued
Once all required testing and verification has been completed our physicians will sign off on either our standardized medical certification format or a specific format or form as required by your destination or organization
- 15 Minute Follow Up Appointment
- Attestation of test result delivered by our clinic

* Certificates issued on non-standard formats (such as a form provided by an embassy) are $95
What Do I Need To Provide Prior To My Visit?
Send an email to [email protected] and provide the details below:
- Medications you are currently taking
- Vaccine History
- Attach to your email the form that needs to be filled out or details of what needs to be written in your modified letter.
- You will most likely require a TB Test which can be performed during your consultation
